Summer Schedule Form

Please complete this form to the best of your ability in order to let us know your summer attendance and scheduling preferences.

Note that we will continue to offer teletherapy sessions and can provide sessions at select camp programs. We will also be offering summer groups and intensive sessions. Learn more about our intensive speech-language therapy offerings here.

Soon after we receive your response, we will E-mail you a proposed summer schedule. Time slots will be offered as forms are received, and they tend to fill up fast, so please respond as soon as possible.
Check off all weeks below that you are in town and planning to attend speech sessions this summer. For each week that you will be in town, indicate any/all days and timeframes* when your child could attend speech sessions.

*Summer sessions will still be 50 minutes each, within the timeframes listed below.
